Freight Policy

Accurate Forms & Supplies ships all products by FedEx or UPS ground services within the Continental United States.  If next day delivery is required please note under special order instructions on order form.  Please note, we cannot ship Second Day or Next Day Air to the following:  P.O. Boxes, APO/FPO addresses, Puerto Rico, Guam or other United States protectorates.  For these shipments Accurate Forms & Supplies will ship by U. S. Mail.  For APO or FPO addresses, please enter APO or FPO in the City field and one of the following two-letter codes in the State field: AE for Armed Forces Europe, Middle East, Africa, and Canada;  AA for Armed Forces Americas; and AP for Armed Forces Pacific.  All instock orders placed by 3:30 p.m. CST, Monday through Friday, will be shipped same day.  Instock orders placed Saturday or Sunday will be shipped Monday.  

Return and Credit Policy

All credit claims (damaged product, short shipments, etc.) must be requested no later than 15 days from date of shipment.  All requests for proof of delivery must be received within 90 days of date of purchase.  Returns on defective products must be made within 180 days of purchase date.  Requests for return authorization is required on all items returned.  To process a credit claim or return:

•              Call Toll-Free 1-800-777-0072 to receive return authorization.

 •              If item is to be returned you may send the product back to Accurate Forms & Supplies or a Return  Label will be sent to pick the product from you.

 •              All returns must be in original packaging with all parts included.

 •              There will be a 15% Restocking Charge on all returns of good product.

 Sales Tax

 If order is shipping to Texas, applicable sales tax will be added to final invoice.
